Who you are
You are a registered nurse with a specialization in cardiology and heart failure, equipped with strong clinical judgment and critical thinking skills. You excel in providing patient-centered care and have excellent interpersonal skills to effectively communicate with patients and the clinical care team. Your ability to track and trend vital signs and assess patients virtually is essential in this role. Bilingual candidates, particularly those fluent in English and Spanish, are highly preferred, as you will be working with a diverse patient population.
What you'll do
In this role, you will remotely monitor and manage patients with heart failure, utilizing advanced technology to track vital signs and assess patient conditions. You will collaborate with the clinical care team to coordinate care and ensure high standards of clinical practice. Your responsibilities will include documenting patient interactions and proactively assessing trends in patient data to enhance care quality. You will also be required to work from home in a HIPAA compliant environment, ensuring patient confidentiality and data security.
