Who you are

You are a compassionate individual with a strong commitment to social justice and a desire to empower individuals facing systemic injustices. You have a background in case management or a related field, and you understand the importance of harm reduction in supporting individuals who use substances. You are skilled in conducting outreach and building rapport with clients, ensuring they feel supported and understood throughout their journey. You are dedicated to helping clients achieve their goals, whether that involves improving their health, securing housing, or accessing social services. You possess excellent communication skills, allowing you to effectively collaborate with various stakeholders, including healthcare providers and community organizations.

Desirable

Experience working with marginalized populations or in a healthcare setting is a plus. Familiarity with health services related to substance use treatment, behavioral health, and HIV/HCV/STI prevention will enhance your ability to support clients effectively. You are adaptable and open to learning, ready to embrace the challenges that come with this rewarding role.

What you'll do

As a Case Manager I at GLIDE, you will provide comprehensive, client-centered case management with a focus on harm reduction. You will manage a dedicated caseload, developing individualized care plans that prioritize client-identified goals. Your responsibilities will include conducting outreach to engage clients, facilitating access to a full spectrum of health services, and providing support through counseling and support groups. You will work closely with clients who are newly diagnosed or have fallen out of care, helping them navigate the complexities of the healthcare system. Your role will involve regular follow-ups to ensure clients remain engaged in their care and receive the support they need to thrive.

You will collaborate with a multidisciplinary team to coordinate services and advocate for clients' needs. This may involve connecting clients with primary care, behavioral health services, and substance use treatment programs. You will also play a key role in educating clients about available resources and empowering them to take charge of their health and well-being. Your work will directly contribute to GLIDE's mission of creating a just and loving community, mobilized to alleviate suffering and break the cycles of poverty and marginalization.
