Talkspace is hiring a Behavioral Health Care Manager to support and coordinate mental and physical health care for patients. This role requires strong clinical collaboration and communication skills, and is fully remote based in New York.

Who you are
You have experience in behavioral health care coordination and are passionate about improving mental health outcomes. Your strong clinical collaboration and communication skills enable you to effectively support patients and work closely with medical providers and other mental health professionals. You thrive in a diverse and inclusive environment, and are excited to contribute to a mission-driven organization focused on making therapy accessible to everyone.
You understand the importance of patient care coordination and are skilled at serving as the primary point of contact for patients enrolled in the Collaborative Care program. You are adept at managing an assigned caseload, ensuring that patients receive the mental and physical health support they need. Your ability to participate in regular consultations with Psychiatric Consultants and communicate treatment recommendations effectively is a key strength.
What you'll do
In this role, you will be responsible for supporting and coordinating the mental and physical health care of patients within the Collaborative Care model. You will work closely with medical providers and the broader care team to ensure that patients receive comprehensive care. Your responsibilities will include serving as the primary point of contact for patients, managing their care coordination, and participating in consultations to discuss treatment recommendations. You will also play a vital role in fostering a diverse and inclusive workplace culture, ensuring that all employees feel valued and respected.
