What you'll do

As a Care Navigation Guide, you will be responsible for curating relevant provider lists for our members who require care. This involves regular outreach to network providers to ensure that the options provided are optimal and readily available. You will collaborate with the Care Navigation Team Lead to refine processes and improve the member experience. Your role will also include discovering and communicating trends to leadership, which will help in enhancing our services. You will utilize all available resources to navigate complex care requests, ensuring that members receive personalized and impactful options that meet their specific needs.
