You possess excellent communication skills and are comfortable working in a remote setting.
What you'll do
As a Care Navigation Guide, you will be responsible for curating relevant provider lists for our members who require care. This involves regular outreach to network providers to ensure that the options provided are optimal and readily available. You will collaborate with registered nurses on complex cases to effectively route care and improve processes based on trends you discover. Your role will be pivotal in enhancing the member experience by providing impactful personalized options that address specific care requests while adhering to service level agreements.
